What is cold laser therapy and how does it work?

Cold laser — also called low-level laser therapy (LLLT) or photobiomodulation — uses specific wavelengths of red and near-infrared light to penetrate the skin and stimulate cells at the mitochondrial level. The result is increased ATP production, faster tissue repair, reduced inflammation, and modulated pain signalling. It doesn't generate heat, which is why the laser is described as "cold."

Is it really painless?

Yes — patients feel nothing or, at most, a very mild warmth at the contact site. There are no needles, no electrical sensation, and no popping or pressure. It's one of the most comfortable treatments we offer, which makes it ideal for highly painful or acute conditions where other techniques would be too aggravating.

What conditions does it treat?

Cold laser is well-evidenced for tendinopathies (tennis elbow, Achilles, rotator cuff), plantar fasciitis, joint arthritis, post-surgical wound healing, sprains and strains, nerve pain, and TMJ. It's also used for chronic neck and low-back pain. If the issue involves inflammation, slow tissue healing, or pain that's been stuck, laser is usually worth trying.

How is it different from heat or ultrasound?

Heat and ultrasound primarily warm tissue to relax muscles and increase blood flow. Cold laser works on a different mechanism — it delivers light energy directly into cells to drive repair and reduce inflammation, without heating the area. That makes laser useful for acute injuries (where heat would be contraindicated) and for deeper structures heat can't really reach.

How long is a typical session?

The laser portion itself is usually 5 to 15 minutes per area depending on tissue depth and condition. When used as a standalone treatment, expect around 15 to 20 minutes in the room. When integrated into a physiotherapy session, it adds 5 to 10 minutes to your appointment.

How many sessions before I notice results?

Most patients notice a change within 3 to 6 sessions, scheduled 2 to 3 times per week early on. Acute issues sometimes respond after the first or second visit; chronic conditions may need 8 to 12 sessions for full benefit. If you haven't seen any change by visit four, we reassess the plan honestly rather than push more of the same.

Are there any side effects?

Cold laser has one of the cleanest safety profiles in physical therapy. Some patients report a very brief, mild increase in symptoms in the 24 hours after the first session as inflammation shifts, but that settles quickly. There's no tissue damage, no burning, and no recovery period — you can return to normal activity immediately.

Is it safe for everyone?

Cold laser is safe for most people, but we avoid it directly over active cancer sites, the abdomen during pregnancy, the thyroid, and over recent steroid injections. Patients with photosensitive skin conditions or those on certain photosensitising medications need a quick screen. We review all of this during your initial assessment so we never apply laser where it isn't appropriate.

Will it be covered by my insurance?

When laser is delivered within a physiotherapy or chiropractic appointment, it's covered under those benefits — and we direct-bill Manulife, Sun Life, Canada Life, Green Shield, Desjardins and more. Some plans also reimburse standalone laser sessions; we confirm coverage with your provider before booking so there are no surprises.

Can I do laser by itself, or must it be part of physiotherapy?

You can do either. Some patients book standalone laser series — particularly for skin healing, scar tissue, or arthritic joints — while most pair it with physiotherapy or chiropractic so the cellular healing happens alongside manual therapy and exercise. Both approaches work; your practitioner will recommend the right fit for your goals.

What should I expect at my first appointment?

A short health history and a focused assessment of the area being treated, then your first laser session. You and your therapist wear protective eyewear, the laser head is moved over the area in a structured pattern, and you'll feel essentially nothing. You'll leave with a clear treatment plan and an expected timeline.

Are at-home laser devices as effective?

Generally no. Consumer devices use much lower wavelengths and power outputs than clinical-grade lasers, so they don't penetrate tissue to the same depth or deliver the dose research shows is effective. They can be useful for very superficial issues or maintenance, but they won't match in-clinic treatment for tendinopathies, joints, or deeper structures.
