What are custom orthotics?

Custom orthotics are prescription insoles moulded to the exact shape and mechanics of your feet. Unlike off-the-shelf inserts, they're built from a 3D cast and a biomechanical assessment, so they correct how your foot loads through every step. At Physio2Health they're prescribed and fitted by Dr. Marsha Black and manufactured by leading Canadian labs.

How is the gait and biomechanical assessment done?

Dr. Marsha takes you through a full posture and joint screen, then watches you walk to see how your foot strikes, rolls and pushes off. She checks arch height, ankle mobility, knee tracking and pelvic alignment because problems higher up the chain often start at the feet. The findings are explained on the spot, and if orthotics are right for you, a foam-box cast is taken in the same visit.

Who actually benefits from custom orthotics?

Anyone whose foot mechanics are driving pain — chronic heel or arch pain, recurring knee or hip strain, low-back discomfort that worsens through the day. Runners, nurses, retail and trades workers, and patients recovering from foot or ankle surgery all benefit, as do people with diabetes who need pressure offloading. If your feet feel tired before the rest of you, that's usually the signal.

What conditions can custom orthotics help with?

Common indications include plantar fasciitis, flat or high-arched feet, bunions, Morton's neuroma, Achilles tendinopathy, shin splints, patellofemoral (knee tracking) pain, IT-band syndrome, sciatica linked to pelvic tilt, and postural overload from prolonged standing. How long does the casting and fitting process take?

The initial assessment and foam casting takes about 45 to 60 minutes in one visit. The lab then manufactures your orthotics, and the finished pair is typically ready in 2 to 3 weeks. You return for a short fitting appointment so Dr. Marsha can check the fit in your actual shoes and make any tweaks before you take them home.

How long does a pair of custom orthotics last?

Most patients get 2 to 3 years out of a pair with daily wear. Lifespan depends on your weight, activity level, and the materials chosen — runners and tradespeople tend to wear them out faster than office workers. Tell-tale signs it's time for a new pair: visible compression of the shell, worn-through top covers, or the original symptoms quietly creeping back.

Are custom orthotics covered by insurance?

Yes — most Canadian extended health plans cover custom orthotics, typically once every 1 to 2 years per person. We direct-bill Manulife, Sun Life, Canada Life, Green Shield, Desjardins and others where the plan allows. We'll confirm your coverage and required paperwork (biomechanical exam report, lab invoice, gait analysis) before you commit, so there are no surprises at reimbursement.

Can I wear my orthotics in any shoes?

Most styles transfer well between running shoes, walking shoes and work boots — anywhere the existing insole can be removed. For dress shoes, low-profile or "fashion" orthotics are made on a slimmer shell, and for sandals or flats we can prescribe a sandal-specific build. Tell Dr. Marsha which footwear matters most to you and the prescription is tailored around it.

How long does it take to break them in?

Plan on a 2-week graduated break-in: start with 1 to 2 hours on day one and add an hour or so each day. Mild arch awareness, calf tightness or a "different" feel through the hips is normal as your body adapts. By the end of the second week most patients can wear them all day comfortably — if anything still feels off, call us and we'll adjust.

Do I need a doctor's referral?

You don't need one to book the assessment — Dr. Marsha can prescribe orthotics directly as a chiropractor. However, some insurance plans require a referral from your family doctor before they'll reimburse you. Check your benefits booklet, and if a referral is needed we'll let you know exactly what wording your physician should include.

How much do custom orthotics cost at Physio2Health?

The full gait analysis, biomechanical assessment, foam casting, custom-cast orthotics and fitting appointment is $500 — one transparent price, no add-ons. Most extended-health plans cover the majority or all of that amount, and we direct-bill where your insurer permits so you only pay the gap (if any) at the desk.

How do I know it's time for a new pair?

Look for flattening or cracking of the shell, a worn-through top cover, the return of old foot, knee or back symptoms, or a noticeable change in your gait. If your shoes are wearing unevenly again or you've had a major change in weight, activity or pregnancy, those are all good reasons to book a re-assessment. Most plans renew coverage every 1 to 2 years.
